The foregoing responses are true and accurate to the best of my knowledge and belief.  I understand and agree that the information about me contained in this questionnaire, examination and treatment by Mike Wiegele Helicopter Skiing health professionals and staff may be shared among the guides and on-site doctors as necessary to provide appropriate advice and treatment, if necessary, and quality assurance.
